At a 16 week scan, gender can be confirmed on ultrasound to 99%+ accuracy provided baby is in a co-operative position. The foetus measures around 12cm crown-to-rump and looks unmistakably human, recognisable facial features, four full limbs, plenty of motion on screen. The placenta is doing its job in earnest, the heartbeat is strong, and many mums start to notice the first flutters of movement around this time. What's Happening at 16 Weeks
At sixteen weeks your baby is roughly avocado-sized, about 12cm from crown to bottom and weighing in around 100 grams. The pace of growth has eased slightly after the breakneck first trimester, but the refinement carries on. Skin is paper-thin and translucent, the network of blood vessels visible just beneath. Lanugo, the soft downy hair that will cover most of the body, is beginning to appear. Eyebrows and eyelashes are forming. The bones, soft cartilage only weeks ago, are quietly hardening into the skeleton baby will be born with.
The senses are coming online too. Baby can pick up muffled sounds from outside the womb, your voice, music, the everyday rumble of traffic on Great North Road if you happen to live nearby, and from here, hearing develops at pace. Light registers through the fused eyelids despite the eyes themselves not opening until around 28 weeks. The mouth has started to suck, hands grasp, and there's a steady stream of motion on screen: kicks, rolls, hiccups, the odd thumb in mouth. Most first-time mums won't feel these movements yet, quickening usually arrives between 18 and 22 weeks for first pregnancies and a touch earlier the next time around, but the ultrasound shows everything.
The reproductive anatomy has fully differentiated by now, and on a good-quality 2D scan it shows clearly. That's why 16 weeks is the gestation most sonographers recommend for a gender determination scan: the genital tubercle is unambiguous, but baby isn't yet so big that positioning starts to become its own problem the way it can in the third trimester.
Gender Determination at 16 Weeks
The single most common reason parents book a 16-week appointment is to find out whether it's a boy or a girl, and 16 weeks really is the gestation that makes that call straightforward. Earlier at 12 weeks the genital tubercle has formed but not yet differentiated enough for confident identification (around 70–75% accuracy, which most sonographers, us included, consider too soft to commit to). By 15 weeks accuracy climbs to roughly 95%. By 16 weeks, with an experienced sonographer and a co-operative baby, you're at 99%+.
The variable is, again, position. If baby is lying with legs crossed, sitting bottom-down, or with a hand carefully placed, the genital region can disappear. When that happens we'll try a few simple repositioning techniques first, a short walk around the clinic, a sip of cold water, a turn onto your side, and if baby still has other plans, you're booked back in for a free recheck inside two weeks (Mon–Fri).
If positional uncertainty isn't an option you can live with, the DNA gender blood test from 8 weeks is the workaround: 99% accurate, indifferent to how baby happens to be lying, and available from much earlier in pregnancy.
